Payment for treatment(s) not included in the plan is due at the time of service.
Our membership plan does not cover procedures completed by specialists outside our practice.
This is an annual plan that requires renewal every 12 months, regardless of whether the benefits have been used. Any unused benefits at the end of the plan period will expire.
